The Case Manager is responsible for building and fostering professional relationships with individuals, family members, and members of the team to maximize the individual’s human dignity, community integration and opportunities for self-direction.

As a Case Manager, you will build and foster relationships with individuals and their families to maximize human dignity and community integration. Responsibilities include monitoring support plans, participating in planning meetings, coordinating assessments, and providing personal care.

Assist in running various recreation programs, provide support to participants and caretakers, and take leadership responsibility in emergency situations.

Respite workers have the opportunity to give the parents/guardians of the participant a break from providing care for their participant. Respite Provider will be responsible for assisting the individual with tasks to be accomplished within their scheduled hours.
